  • Support board material content creation
  • Other duties as assigned

  • Active medical license for market-specific state or states

  • 7 or more years’ experience in clinical practice with outpatient or home-based practice as a component of practice, value-based care experience preferred
  • Experience in one or more clinical leadership roles including but not limited to medical director, service line leader, division chief, or department chair, and experience working with and coaching APPs and other clinicians
  • Experience using KPIs and data to improve quality and clinical outcomes for patients and monitor cost of care
  • Track record of delivering clinical outcomes that decrease patient utilization for a complex patient population.
  • Comprehensive understanding of the success drivers of value-based care, not limited to risk adjustment, HEDIS, and utilization.
  • An inspirational leader who is laser-focused on leading a team that delivers clinical outcomes aligned with demonstrating medical expense reduction
  • Experience with advanced kidney disease and ESRD clinical management a plus
  • Innate curiosity and drive to deliver measurable impact in a start-up healthcare entity
